School is starting back and HH is here to serve the community!

HopeHealth is now on campus in the Health Connections building at Lake City High School:

  • Every Tuesday and Thursday   8:30 a.m.-3 p.m. (beginning August 16, 2022!)

How does this help you?

Included with a well-child visit, your child can get:

  • The Tdap vaccination that is required by law for every child going into 7th grade
  • A sports physical for the upcoming school year
  • Caught up with CDC recommended vaccines

**We can also see sick children, faculty, and staff during these hours**

What do you need to do?

  1. Fill out the front and back of the HopeHealth consent form
  2. Return the form to the school office, the school nurse
  3. Send the form in with your student or bring it with you to the appointment

HopeHealth School-Based Clinics
Services include: basic primary care, screenings, immunizations, behavioral
health, and referrals as needed to students, student families, and staff.

How this helps you

To become a patient
