How to Apply for Pre-K/Child Development Program

for the 2021-2022 School Year


Pre-Registration Steps:

#1 Online Application:

  • Complete the FSD3 Pre-K/Child Development application online in the PowerSchool Portal.
  •  Parents are encouraged to apply early.

#2 Dial-4 Screening:

  • After an application is complete, parents will schedule a DIAL-4 child screening.
  • A DIAL-4 screening schedule will be emailed to each applicant starting March 5, 2021.
  • Parent/Guardian will choose: Time, Location and Date.

*DIAL-4 screenings are required and will take about 45 minutes to complete.

*More DIAL-4 Screening directions will be emailed once the parent/guardian completes the screening sign up process.

#3 Notification of Status - Once DIAL-4 screening is complete and all documentation including the Immunization Record are received, qualified and/or waitlist notifications will be emailed and mailed to parent/guardian by April 30, 2021. Please check your email often.

  • Round 2 qualified/waitlist notifications will be sent May 30, 2021.

All accepted seats will be considered pending until the student's documents and residency is verified by the school.

Important Documents You Need:

  • Photo Identification (driver’s license/state identification)
  • State-Issued Birth Certificate (long form is required)
  • Applicants must be 4 on or before September 1, 2021.
  • Proof of Income or Medicaid Card
  • Updated Immunization Certificate
  • Proof of Residency (current electric bill displaying service address and a billing date)
  • Address on the electric bill must match photo identification. (If the electric bill is not in your name, a notarized document verifying residency must be completed.)
  • Social Security Card

It is a tool to be used by instructors to identify a students reading level, accuracy, fluency, and comprehension. Once levels are identified, an instructor can use this information for instructional planning purposes.
